Previously known as the Home Care Package (HCP) program, the Support at Home Program is an Australian Government initiative that helps older Australian’s stay independent, safe and well in their own homes. This program provides funding for in-home care services including allied health therapy services based on your needs.

All our services can be provided under the Support at Home program. We are a preferred provider for dozens of Aged Care providers and are experienced in providing appropriate care within a budget to meet your needs.

You may be eligible for the Support at Home Program if you:
- Are aged 65+ (50+ for Aboriginal or Torres Strait Islander peoples)
- Have been assessed by My Aged Care as needing support to remain living safely at home
- Require allied health services as part of your care plan

How accessing the Support at Home program works
1. My Aged Care Assessment
Your journey begins with an assessment through My Aged Care. An assessor will determine the supports you need to stay independent at home.
2. Creating Your Support Plan
If allied health services are recommended, they will be included in your Support at Home plan.
3. Choosing Your Provider:
You have the freedom to choose an allied health provider, like Lifecare Homecare, to deliver the services in your support plan.
4. Tailored Therapy Plan
Our team will visit you to understand your specific needs and goals. We’ll work with you and your care manager to create an individualised therapy plan that aligns with your support plan.
5. Delivery of Services
We offer flexible care where it suits you whether at home, in community centres, at gyms, hydrotherapy settings, or in Residential Aged Care Facilities.
6. Ongoing Support
We’re with you every step of the way. Our allied health professionals provide practical, goal-focused care to help you stay mobile, manage health conditions, and improve your quality of life.
